I can hardly believe the end of the school year has arrived. As you prepare for summer vacation, I wanted to send a friendly reminder regarding medication pick up, kindergarten and 6th grade requirements for 2024-2025 and dental requirements
 
If you have not turned in a completed physical with immunizations for your child who will be in kindergarten or 6th grade during the 2024 -2025 school year, please do so now. If you have not already made an appointment for your child to receive their kindergarten or 6th grade physical and required vaccines, please make the appointment today as appointments fill up fast. 
 
End of the year medication pick up:
We will not send any type of medication (except for inhalers) home with your child on the last day of school (Monday, June 3rd).  
 
Please plan to pick up your child’s medication in the office by (Monday, June 3rd). 
 
Medication which is not picked up by a parent/guardian by Monday, June 3rd, will be disposed of. 
 
2024 – 2025 Kindergarten requirements:

  1. A new physical examination including a completed immunization record (dated on or AFTER August 15th, 2023) is required for entry into kindergarten. 
  2. Vision exam is required for entry into kindergarten (dated within one year of school entry).
  3. Dental exam is required for entry into kindergarten (dated on or after 11/15/23)

2024 – 2025 6th grade requirements:

  1. A current physical examination including a complete immunization record is required for entry into the 6th grade (dated on or AFTER August 15th, 2023). Please do not turn in a physical dated BEFORE 8/15/2023. 
  2. Dental exam is required for 6th grade (dated on or after 11/15/23)

Dental Requirements:
Students in kindergarten, 2nd and 6th grade also must have had a dental exam within 18 months of May 15 of that school year (no sooner than 11/15/2023). Dental Examination Form
